Group Lifestyle Balance Program™                            

This program is a 22 week program, led by certified coaches, for people who have been identified with, or are at risk for prediabetes and/or metabolic syndrome.  It is offered at various locations throughout the region. 

 

The program is offered in groups of 10 to 12 participants and focuses on helping participants adopt and maintain lifestyle skills, including weight loss, nutrition and physical activity. 

  

The program is FREE to attend and is funded by the Ontario Ministry of Health and Long Term Care (MOHLTC).  It is based on the University of Pittsburgh Diabetes Prevention Support Centers’ program.

The program consists of 3 phases:

 

    Phase 1: Core Phase

  • 12 sessions over 12 weeks

    Phase 2: Transition Phase

  • 4 sessions that can last from 4-8 weeks

    Phase 3: Maintenance

  • 6 sessions lasting a minimum of 12 weeks but can be spread over as many as 24 weeks
