What is Self-Management?     

Self-management is defined as the active, engaged participation of individuals in achieving their best health and wellness. This involves gaining the confidence, knowledge and skills to manage physical, social and emotional aspects of life, in partnership with healthcare providers and community supports.

 

Self-Management workshops are designed to support you with getting the most out of life while living with a chronic or long-term condition. Our workshops are offered for FREE in region of Waterloo Wellington. Virtual and in-person programs are available. These workshops are designed to support you with getting the most out of life while living with a chronic condition. The following programs are offered for FREE in the region of Waterloo Wellington.

 

Take Charge! Chronic Conditions

 This 6-week workshop is for anyone living with a chronic (ongoing) health condition. It is led by certified Peer Leaders.

Take Charge! Chronic Pain

This 6 week workshop is for people struggling with chronic or persistent pain.  It is led by certified Peer Leaders.

 

Take Charge! Diabetes

This 6- week program was developed to support individuals living with Type 2 Diabetes and their families to help manage their diabetes.

 

Anxiety Skills & Strategies

This 3-week program you will have the opportunity to explore a variety of long and short self-calming techniques, along with behaviours that can help prevent escalation of anxiety.

 

Better Sleep

This program will help you learn strategies to relax your mind and body, to decrease insomnia, and to fall asleep or back to sleep easier.

 

Craving Change®

This 4-week program helps you understand why you eat the way you do.  It is led by health care professionals certified in the program.

 

Dial It Down- Relaxation and Stress Management

This program helps you develop strong skills in releasing body tension, calming and focusing the mind.

 

Getting The Most From Your Healthcare Appointment© 

This program helps you learn how to ask the right questions of your doctor or health care provider so you understand and feel confident in taking an active role in your own healthcare. 

 

Managing Diabetes in Ramadan

A free virtual classroom for persons with diabetes and their caregivers to learn how to manage diabetes safetly while fasting during Ramadan

 

Perimenopause Unveiled

Learn about the changes occurring in your body, and manage symptoms as you transition into menopause.

 

Powerful Tools for Caregivers

This 6- week interactive workshop series is designed to help caregivers develop a wealth of self-care tools.

 

Relaxation Techniques

This 1- hour session will help participants learn skills to unwind from stress.

 

Stop Worrying!

This 2- hour workshop provides practical strategies to curtail unhelpful worrying and decrease its negative impact on mood & performance.

 

 

Become a Leader

Do you enjoy teaching and public speaking? Joing us for a leader training workshop to become a "Peer Leader" for our self-management workshops.

 

 

Take Charge Leader Training

This training is a 4 day training to become a certified trainer.
